HEALTH INSURANCE floater plan for me considering that I am a smoker ?

Dear sir,

I am 54 yr old. I do not have any kind of insurance. My family includes my wife and two children aged 20 and 13 yrs respectively.Kindly suggest an appropriate HEALTH INSURANCE floater plan for me considering that I am a smoker.
