Did not get full claim amount in ICICI Prudential Health Saver Policy

I have health saver policy of icicipru life since 5 years, paying premiums regularly. I got operation of cataract of both eyes done in span of 1 week. Insurance co. given me rs 20000/- out of total bill rs 43364/- acc. clause is 8.8.18 Actually that clause is for first two years. please give information about this.
