Where to get the Value of Indexation when Gold ETF is sold after 1.5 Years and which ITR Form to fill?

Dear Experts,

I had bought 3 units of GOLD ETF’s during March 2013 at a price of 8356/- and sold in October 2014(nearly 1.5 years) at a price of 7697/- (Loss of  659).  I have some questions regarding this:

1. AFAIK I need to fill ITR-2 while filing IT Returns for FY 2014-15.  Am I Correct?

2. Calculating Cost Inflation Indexed(CII) purchase price=((Purchase Price) x (selling price-purchase price))/Indexation

CII adjusted LTCG = Selling Price – CII Purchase price

and 20.6 % tax on CII adjusted LTCG

Is this formula correct? Also where would I get the value of Indexation? Any link for this?

3. How do I declare this in ITR-2?
