Parent’s health information wrong in Policy document

Last year I took a term insurance from Kotak and now when I was reviewing policy document I found that I had incorrectly stated my father’s health details. He is actually suffering from diabetis and blood pressure. I don’t remember whether I forgot to mention this or agent incorrectly filled it up. Can I go back and get this detail corrected? I went through a medical check up before Kotak issued policy to me?

I’ve paid two premiums so far, am I’m 30 now. What are the options available to me to get this corrected / get a new policy with correct details.
