Form 16 – E filing Query

Hi,
i want some help with relation to e-filing of income tax.
In jan 2013, I changed my job and hence now I have got 2 form-16.
now income from both the companies I need to show for filing returns..

now, the questions I have are listed below:

1. Do I need to simply add respective sections in both the form -16  and write the

total in the itr-1?

2. There is exempt of 15000 for medical insurance. Now both of my form-16 shows

15000 exempt of medical insurance (against the medical bills provided). So can I add

this income to show the 30000 in section 80D

3. I have paid some premium in the month of march is not reflecting anywhere in the form 16 so can I show this amount to get exempt?
