How to withdraw from my mutual funds when the bank account has changed ?

I have purchased 4 different mutual funds in . Now I have different bank accts, changed address, phone number. how 2 redeem money from mutual funds. I have very less idea abt mutual funds. I only know the name of mutual fund and folio numbers. (HDFC top 200 folio no. 4410899/43), HSBC prog theme 906766/67) rel div power (40860393191/ 40860390772 and UTI inf adv fund series-1 ) growth plan fol no. 509208082531. The name have been changed at later stage(may be). THANK U

    You can submit form to the respective AMC to change the bank account, phone, mail id.
