Regarding Claim of LTA and Medical Reimbursement when its more than the employer benefit

My employer provides me Medical Reimbursement of Rs. 5000/- Per Annum and LTA of Rs. 5000/- Per Annum. I want to clarify myself that-

A) Suppose for a year my medical expenses are Rs. 7200/- PA( more than the amount given by employer). Then do I have to pay that extra Rs. 2200/- from my pocket?
B) Suppose for a year my LTA expense is Rs. 8000/-. Then in this case also do I have to pay that extra Rs.3000 from my pocket?

Please clarify

  1. Yes, thats correct . You will be bearing the extra cost yourself . The company will just return you the amount upto the limit.

    1. Thanks for the reply. I also want to know that LTA can be taken twice in a block of four years. Suppose i reimbursed my LTA for first two years in a block. Then in this case do i get the LTA amount for the remaining two years included in my salary?

    2. In case of Family Medical Insurance(if provided by the employer), who will be paying the premium of insurance, the employee or the employer? And in case the employee or dependent is hospitalized for any illness and the hospital bill is say 50,000. Then this will be borne by the employee or employer?
