Filling Name while Buying Term Insurance

I am facing an issue , please help!

my wife name is R.ANITHA RUDHRAJU. R stands for ramalingaraju (father name), anitha is first name and rudhraju is family name. In certificates name is simply written as R.ANITHA RUDHRAJU.But when it comes to passport firstname-ANITHA,middlename-

RUDHRAJU,lastname-RAMALINGARAJU.

when i go for icici term insurance they ask only two things firstname and lastname.i have contacted them several times via mail,phone,etc no use.

In general how to write the above name in firstname and lastname any legal obligations in both names . and space are not allowed

    Just try to add middle name in first name box. If it does not accept that, just fill up the online form as it accepts. At the time of signing of application form and submitting the required documents, write the application for the name you required on policy bond and inform ICICI Prudential Customer care by email also.
