Documents needed for surrendering LIC policies

Hello,
I would like to surrender my LIC money back policies.
I am carrying the original policy documents with me.Do I need to take any other documents(like id proof) to the LIC office during surrender?

    Dear Phani, Although not required normally but to be on safer side, please carry your PAN Card or any other ID proof also with you.

    Hi Phani,

    You need to submit one surrender form ( with revenue stamp signed by policy holder) You can get this from any LIC agent or from LIC office or you can download from internet.
