Fraud in Life Saver Policy from Aviva

I have a life saver plus policy no.LSP1726945 of AVIVA.
The policy in question shall convert into auto foreclosure on 27.10.2012 as per Aviva UIN No.122L037V01 Dt.28.08.2012.Its surrender value in august was Rs.124295.00 though Rs.150000.00 have been paid by me since 26.11.2009. There are a few disturbing things that came to my notice, regarding the same policy. 

Firstly the policy was sold to me  by HDFC Bank manager at Solan Mr. Prashant Goel. I was assured a better return on the policy and I bought it trusting him.
The issues I have with the policy are 
1. The Medical Reports have been forged.
I have never been examined by any doctor, but the reports state that a doctor paid me a visit.Aviva says that I paid a visit to the doctor at Pachkula on the contrary.
Many details in my report are not accurate, I have certain old medical conditions which have not been mentioned in my reports. 
2. There is difference in Height/ Weight.My date of birth is also wrong in Medical Examiners report. 

Talking about the Mr.Prashant Goel he did co operate when I brought this fact in his notice. Even he confirms that no body has ever approached him from Aviva\\\’s any agency or empanelled doctor for any medical examination to be done. Even the advisor Mr. Manoj Bariana has also confirmed that, he is also not aware about the medical being done. The medical could not be done without the policy advisors knowledge. The copy of this communication is also attached at the end. 

I believe that it was HDFC that sold this policy to me and i hold the bank liable for this forgery. There is enough proof that there are a lot of dubious  aspects to this issue. I did notify the Aviva head office the response i got from there was not at all to my satisfaction. I think this is not fair. The policy was bought on the banks goodwill and I would not want to be misguided or cheated. I would like a thorough probe in this regard.

How to proceed to teach Aviva and HDFC bank a lesson so that they do not cheat the clients in future.

    I GOT REPLY.AVIVA SAYS THAT I WALKED IN FOR MY MEDICALS EVEN THOGH MEDICAL REPORT SAYS ITWAS HOME VISIT BY THE DOCTOR.MY PAN CARD HAS A DIFFERENT DATE OF BIRTH THAN MEDICAL REPORT.SINCE MY BIRTH DATE WAS WRONG IN FORM THE DR.COPIED THE SAME.THIS ALSO CONFIRMS THAT I DID NOT VISIT THE DR.THERE NEVER WAS ANY COMMUNICATION ABOUT THE REQUIREMENT OF MEDICAL.NEITHER ME OR THE BANK MANAGER AND ADVISOR ARE IN THE KNOWLEDGE OF REQUIREMENT OR ANY COMMUNICATION ABOUT MEDICAL .HOW COULD THE MEDICAL BE DONE WITHOUT OUR KNOWLEDGE OR INFORMATION.
    THIS IS SURPRISING.
    KINDLY ADVISE WHAT LEGAL COURSE CAN BE TAKEN KNOW.I SHALL ALSO APPRECIATE IF YOU CAN RECOMMEND A TOP LAWYER TO HANDLE THIS ISSUE.

    I have forwarded this link to the top management of Aviva whom I know personally., you should get some reply from their end hopefully .

    This seems to be a fraud act by the person who sold you this policy . Also didnt you check the policy document and all the things mentioned when you got the policy document ? Seems like you read them after many years !
